Do you need to request an appointment?


Are you or someone you know in need of services? Phone requests can not be accommodated at this time. Appointment request must be made with us via the link below at least 2 days prior to desired date. Please indicate your preferred date, time, and location. We will follow-up by email confirming your request within 48 hours and do our best to accommodate your appointment request.  Please provide your insurance information when you request the appointment.

*To request an online telemedicine appointment, first make a request via the “Book Your Appointment” link.  Indicate that you would like an online session in the comment section. After appointment has been approved, you will go to to attend online session.

*Please provide insurance information in the comment section of the appointment request, so that your insurance can be verified before your first session. 

*Please bring your insurance card to your first session.

*Please verify that your deductible/co-insurance has been met before your first session.  If your deductible is not met, you will responsible for the entire session.

*Please bring your EAP authorization number to your first session.  Please inform your EAP company that you have selected our practice to provide your sessions before your first appointment.
